Online Enrollment and Account Management
For your convenience, you may now enroll online. Account
Holders may make deposits and take disbursements through our eChecking service. Other features
include the ability to log-in and review account information, calculate contribution amounts and

Education About HSAs and Account Set-up
We are available to attend employee meetings to educate, enroll and establish health savings accounts at
an employer worksite, or on a one-to-one basis with individuals who are self employed.
Account Contribution Management
Employer specific services including reports on contributions and disbursements, reports that distinguish
between employer and employee contributions for tax purposes, and the ability to provide automatic deposits
from multiple sources for each individual employee account.
Record Keeping
Data collection and record keeping of deposits and disbursements. Fremont Bank HSA automatically scans
and archives all Explanation of Benefits (EOBs) and receipts and will provide these documents in the event
of an IRS audit. Notification to account holder if deposits exceed the maximum amount allowed by law and/or
distributions that do not appear to meet the IRS definition of qualified medical expenses.*
Bill Paying Services
Payment of health care service bills (based on Explanation of Benefits) to providers with prior approval
from individual account holders or as a reimbursement to the account holder. Fremont Bank HSA automatically
reviews all Explanation of Benefits (EOBs) and receipts for HSA compliance and verifies that the correct
network discounts have been applied to ensure that account holders never pay more than they should.

Account Statements
Quarterly statements for HSA account holders and employers are provided to keep them well informed about
account activity. This information is also online. Go to the Login section of the website and
access statements using your personal log-in information. If you need help, call us at (800) 554-6145
Monday-Friday from 8:00am to 6:00pm Pacific Time.
