Online Enrollment and Account Management
For your convenience, you may now enroll online. Account
Holders may make deposits and request disbursements through our eChecking service. Other features
include the ability to log-in and review account information, calculate contribution amounts and

Education About HSAs and Account Set-up
We are available to attend employee meetings to educate, enroll and establish health savings accounts at
an employer worksite, or on a one-to-one basis with individuals who are self employed.
Account Contribution Management
Employer specific services including reports on contributions and disbursements, reports that distinguish
between employer and employee contributions for tax purposes, and the ability to provide automatic deposits
from multiple sources for each individual employee account.
Record Keeping
Online data collection and record keeping of deposits and disbursements. HSA Manager allows you to track contributions and use of your Health Savings Account including your receipts all in one place.
Bill Paying Services
Fremont Bank's HSA Manager allows you to pay bills online for eligible medical expenses. We provide informartion and guidance on eligible medical expenses for each transaction.

Account Statements
Monthly statements for HSA account holders and employers are provided to keep them well informed about
account activity. This information is also online. Go to the Login section of the website and
access statements using your personal log-in information. If you need assistance, call us at (877) 478-4472
Monday-Friday from 5:00am to 4:00pm (P.T.).
