There are two ways to sign up for a Fremont Bank HSA, online or by US Mail.

1. Sign Up for a Fremont Bank HSA Online
You can enroll, make your initial deposit, and register for access to manage your account online. You can open your account for only $100.00 plus a $35.00 annual fee with unlimited free use of a debit card and eChecks.

If you would like to sign up online, please make sure you can provide details on all the following information requirements:

• Personal Profile including Driver’s License or Passport Number
• Annual Health Plan deductible and date coverage begins
• Dependents (for family plans)
• Bank Account information including Routing and Account Number

2. Sign Up for Fremont Bank HSA via US Mail
Signing up for an HSA with Fremont Bank is as easy as 1 - 2 - 3! Enrollment kits and other useful materials are available in the Douments & Forms section. Just complete the following:

• Complete and sign the enrollment form and the beneficiary designation form
• Mail to Fremont HSA, PO Box 4556, Hayward, CA 94540-4556
• Include a check for an initial deposit ($100.00 minimum) plus a $35.00 annual fee.
